Terms and Conditions

Last reviewed: Monday, 21 September, 2026 Effective date: Monday, 21 September, 2026

These Terms and Conditions (“Terms”) govern your use of the Mindora Health website (mindorahealth.com.au) and the services we provide. Mindora Health (ABN 35 669 694 277) (“Mindora Health”, “we”, “us”, “our”) is a Mental Health Nurse Practitioner-led service based at Shop 1-33/35 Princes Highway, Dapto NSW 2530, providing in-person and telehealth mental health care across the Illawarra region and New South Wales.

By using our website, booking an appointment, or receiving care from us, you agree to these Terms. Please read them carefully. If you do not agree with them, please do not use our website or book an appointment.

This is not an emergency service

Mindora Health does not provide crisis or emergency care, and our website and services are not monitored around the clock.

If you or someone else is in immediate danger, or life is at risk, call 000 now. For 24-hour mental health support you can call the NSW Mental Health Line on 1800 011 511 or Lifeline on 13 11 14. Do not use our website, email, SMS or booking form to communicate an emergency — these channels are not monitored in real time and are not an appropriate way to seek urgent help.

About our service

Mindora Health is led by an AHPRA-registered Mental Health Nurse Practitioner. Our care model is hybrid: an initial comprehensive in-person assessment, followed by telehealth reviews and follow-up appointments. We operate with no waitlist, and a GP referral is not required (though a referral, where you have one, is helpful and encouraged).

Our services may include comprehensive biopsychosocial assessment and diagnosis, investigations, treatment planning, medication management and deprescribing, and the preparation of reports and letters (for example NDIS, DCJ Housing and Centrelink) where clinically appropriate.

The information on our website is general in nature and is provided for information only. It is not medical advice and is not a substitute for a consultation with a qualified health practitioner. Care recommendations are made only within a consultation, based on your individual circumstances.

Our care is intended to complement, not replace, your relationship with your GP. We encourage you to maintain a regular GP, and — with your consent — we will communicate with your GP to help coordinate your care.

Eligibility

To receive care from us you must be at least [18] years of age [confirm — and adjust if the clinic accepts younger clients under specific arrangements]. You must provide accurate and complete information about your identity, contact details, medical history and current medications. Our ability to care for you safely depends on the accuracy of the information you give us.

We accept clients located in [New South Wales / Australia — confirm the geographic scope for telehealth]. You must be physically located in Australia at the time of a telehealth consultation.

Accepting a new client is always at our clinical discretion. Where your needs would be better met by another service — for example acute inpatient care, or a service we do not provide — we will tell you and, where we can, help point you in the right direction.

Appointments and bookings

You can request an appointment through our online booking form, or by contacting us on 0425 576 017. Submitting a booking request does not, by itself, guarantee a confirmed appointment — your booking is confirmed once we have accepted it and confirmed the time with you.

Your first appointment is an in-person comprehensive assessment. Follow-up appointments are generally conducted by telehealth, unless an in-person review is clinically indicated.

You are responsible for attending your appointment at the confirmed time, and for providing accurate contact details so we can reach you.

Fees and payment

Our fees are advised to you at or before the time of booking. Payment is due [at the time of your consultation — confirm the clinic’s payment timing]. We accept [confirm accepted payment methods]. Card payments are processed by our secure third-party payment provider; we do not store your full card details on our own systems.

Medicare rebates. Some of our services may attract a Medicare rebate where you are eligible and the relevant requirements are met. Any rebate is paid by Medicare, not by us, and eligibility and rebate amounts are determined by Services Australia — not by Mindora Health. We will tell you which items may attract a rebate, but we cannot guarantee that Medicare will approve a rebate in your circumstances. Where a service is not covered by Medicare, or you are not eligible, the full fee applies.

Fees may change from time to time. The fee that applies to your appointment is the one advised to you for that appointment.

Cancellations and missed appointments

We ask that you give us at least [24 / 48 hours — confirm] notice if you need to cancel or reschedule an appointment, so that we can offer the time to someone else. Cancellations made with less notice, and appointments you do not attend (“no-shows”), may attract a cancellation fee of [amount / percentage — confirm]. We understand that emergencies happen, and we will consider genuine, unavoidable circumstances on a case-by-case basis. The cancellation policy that applies is the one advised to you at the time of booking.

Telehealth

Where care is delivered by telehealth (video or phone), you acknowledge and agree that:

  • telehealth is suitable for many, but not all, situations, and we will use our clinical judgement about whether a telehealth appointment is appropriate for you at a given time — we may ask you to attend in person instead;
  • you are responsible for having a suitable device, a stable internet connection and a private, quiet space for your consultation;
  • technology can fail — if a connection drops or the quality is inadequate, we will attempt to reconnect or reschedule, and this does not entitle you to a refund beyond the portion of the service not delivered;
  • you must be located in Australia at the time of your telehealth appointment; and
  • telehealth is not appropriate in an emergency (see This is not an emergency service above).

Prescriptions and medication

Any prescribing is done within a clinical consultation, based on an assessment of your individual circumstances, and in accordance with the Nurse Practitioner’s scope of practice and relevant NSW and Commonwealth medicines legislation.

We do not prescribe on demand or without an appropriate assessment, and we may decline to prescribe, or to continue prescribing, where it is not clinically appropriate or safe to do so. Requests for repeat prescriptions should be made with reasonable notice and may require a review appointment. Certain medicines — including Schedule 8 (controlled) medicines — are subject to additional legal requirements and clinical safeguards, and we manage these accordingly.

Reports, letters and third-party documents

Where you ask us to prepare a report or letter — for example for the NDIS/NDIA, DCJ Housing, Centrelink or another third party — we will prepare it honestly and accurately, based on our clinical assessment and the information available to us.

You acknowledge that:

  • the content of any report reflects our genuine clinical opinion, which we cannot alter to produce a particular outcome;
  • we cannot guarantee how a third party (such as the NDIA or Centrelink) will decide your matter — that decision rests entirely with them; and
  • a fee may apply to the preparation of a report or letter, which is payable regardless of the outcome of any application.

Communication with us

We may communicate with you by phone, SMS and email for appointment reminders, administrative matters and, where appropriate, aspects of your care. Please note that email and SMS are not secure and are not monitored in real time. Do not use them for urgent or emergency matters, and be aware that any information you send us electronically carries the ordinary risks of that medium. If you prefer we not contact you by a particular method, let us know.

Your responsibilities

To help us care for you safely, you agree to:

  • give us accurate, complete and up-to-date information about your health, medications and circumstances, and to tell us promptly if anything changes;
  • attend appointments (or cancel with appropriate notice), and follow the agreed treatment plan or discuss any concerns with us;
  • treat our staff with courtesy and respect; and
  • not use our services or website for any unlawful purpose.

We may decline or discontinue care where a therapeutic relationship cannot safely or appropriately be maintained. Where we do, we will act professionally and, where possible, help you find alternative care.

Website use and intellectual property

The content on this website — including text, graphics, logos and images — is owned by or licensed to Mindora Health and is protected by copyright. You may view and print pages for your own personal, non-commercial use. You must not reproduce, republish, modify or distribute our content for any other purpose without our prior written permission.

You agree to use our website lawfully and not to attempt to interfere with its operation or security, introduce malicious code, or use it in any way that could damage or disrupt the site or another user’s use of it.

Third-party links

Our website may contain links to third-party websites — for example government, professional or support-service pages — provided for your convenience and information. We do not control those sites and are not responsible for their content, availability or privacy practices. A link does not imply our endorsement.

Privacy

We handle your personal and health information in accordance with our Privacy Policy and applicable Australian privacy law, including the Privacy Act 1988 (Cth) and the NSW Health Records and Information Privacy Act 2002. By using our services you acknowledge that you have read and understood our Privacy Policy. Please read it alongside these Terms.

Disclaimers and limitation of liability

We provide our services with reasonable care and skill, consistent with the professional standards expected of a registered Nurse Practitioner. However:

  • no health service can guarantee outcomes. We do not promise or guarantee any particular result, cure, diagnosis, recovery, or the approval of any rebate, claim or third-party application. Where we describe how treatment “may” or “can” help, this reflects clinical possibility, not a guarantee.
  • the general information on our website is provided “as is” and is not a substitute for individual clinical advice.

Nothing in these Terms excludes, restricts or modifies any rights or remedies you have under the Australian Consumer Law or other laws that cannot lawfully be excluded, including the consumer guarantees that apply to the supply of services. To the extent the law allows us to do so, and subject to those non-excludable rights, our liability for any loss or damage arising from your use of our website or services is limited to re-supplying the relevant service, or paying the cost of having it re-supplied. We are not liable for loss or damage caused by matters outside our reasonable control, including failures of third-party technology, internet connections, or third-party decisions (such as those of the NDIA, Medicare or Centrelink).

Professional standards and complaints

Our Nurse Practitioner is registered with the Australian Health Practitioner Regulation Agency (AHPRA) and the Nursing and Midwifery Board of Australia (NMBA), and practises in accordance with their codes and standards.

If you have a concern about your care or your experience with us, please contact us first — most concerns can be resolved quickly and directly. If your concern relates to how we handled your personal or health information, our Privacy Policy explains how to raise it. You may also contact the Health Care Complaints Commission (HCCC) in NSW (1800 043 159, www.hccc.nsw.gov.au) or AHPRA (www.ahpra.gov.au).

Changes to these Terms

We may update these Terms from time to time to reflect changes in our services or in the law. The current version is always available on our website, and the “Last reviewed” date at the top shows when it was last updated. Your continued use of our website or services after a change means you accept the updated Terms.

Governing law

These Terms are governed by the laws of New South Wales, Australia, and you submit to the non-exclusive jurisdiction of the courts of New South Wales.
